Chronic Disease in Murrumbidgee

The Integrated Care Coordination Program (ICC) team works with GPs to help improve access to health services for people with complex health needs.

ICC aims to support the integration and coordination of health services, to deliver improved health outcomes by building cohesive relationships and community awareness of primary health services across southern NSW.

Care Coordinators will:

  • Conduct an initial assessment
  • Coordinate health care advocacy and assistance
  • Have direct engagement with the patient and relevant carers
  • Have direct engagement and case conferencing with GPs
  • Liaise with other local service providers to avoid patients falling through the service gaps
  • Use a team based approach to care planning
  • Support clients to cease smoking
