The Healthy Eating Activity & Lifestyle (HEAL™) program is a 8-week lifestyle modification program that supports people to develop lifelong healthy eating and physical activity habits. HEAL™ consists of 2 hour weekly group sessions over 8 weeks including 1 hour of lifestyle education and 1 hour of low to moderate intensity exercise. HEAL™ education and exercise sessions are facilitated by allied health professionals and is specifically designed for people who are at increased risk of developing lifestyle diseases such as cardiovascular disease and Type 2 diabetes. HEAL™ is also suitable for people seeking to manage their weight, Type 2 diabetes, cholesterol or high blood pressure.
Referrals to the HEAL™ program can be made through a range of primary health professionals:
- General Practitioner
- Allied Health Professional
A referral through a General Practitioner is not essential, however it is strongly recommended. This allows the HEAL facilitator to effectively communicate back to the GP on the participant’s progress and encompass multidisciplinary care.
