Chronic disease management and prevention

Through our fully-funded Chronic Disease Management and Prevention Program we work with General Practices and Aboriginal Community Controlled Health Organisations to provide services where there is no alternative funding or services available.


The program targets people with two or more preventable lifestyle chronic diseases, or who are at high risk of developing a chronic disease. This includes conditions such:
  • Diabetes
  • Cardiovascular disease
  • Respiratory disease
  • Renal disease, and
  • Some cancers

We deliver dietetic and diabetes education services under this program.

How do I access the services?

To access Chronic Disease Management and Prevention services, it is desirable that you will:

  • Have a GP Medical Plan (GPMP) in place, and
  • Have been identified by your GP as having two or more chronic diseases.

Access to all services is by appointment only, and appointments can be made by contacting your nearest location from the list above.
