Chronic disease management and prevention

Through our fully funded Chronic Disease Management and Prevention service, our diabetes educators and dietitians support people with two or more chronic diseases, or who are at high risk of developing a chronic disease. 


This includes conditions such:

  • Diabetes
  • Heart disease
  • Respiratory disease and breathing problems
  • Kidney disease
  • Some cancers

How do I access the chronic disease management and prevention services?

To access these services, you need the following from your GP:

  • A GP Medical Plan (GPMP) 
  • Diagnoses of two or more chronic diseases or identified risk of chronic disease

Access to all services is by appointment only – appointments can be made by contacting your nearest location from the list above.
