How can a diabetes educator help?
If you need support understanding and managing your diabetes, our team of Credentialed Diabetes Educators are here to help by providing specialist services for adults diagnosed with pre-diabetes and type 2 diabetes.
We can support you by providing advice about managing your blood glucose levels, making healthy food swaps and understanding technology and medication.
Who is diabetes management and support for?
Diabetes management and support is for adults diagnosed with type 2 diabetes or at risk of developing diabetes. We take a person-centred approach to your needs – supporting you to understand your blood sugar levels, risk factors, dietary needs, lifestyle changes and how to look after your mental wellbeing in the process.
We also provide Type 2 group education classes that support you to learn to live with diabetes, show where to go for support, understanding nutritional value of foods and so much more. The groups also connect you with a social community to support you through diabetes management.
How do I access?
We currently offer four types of services for people with diabetes:
- Fully funded services for First Nations people:
- In Bathurst though the Indigenous Chronic Disease Clinic (a GP referral is required)
- In Coonamble and Wellington through the Medical Outreach Indigenous Chronic Disease program
- Through the NDIS if funding for diabetes education is required as part of your NDIS plan
