The Psychosocial Physical Health Pilot Program connects you with health professionals who will support you to achieve physical health goals to help enhance your overall wellbeing.
Our team will provide one-on-one coaching and work in partnership with you, your family and carers (where possible). This is not a clinical service, but a community-based program providing short-term, low intensity individualised support to achieve your physical health goals.
We also provide group health wellbeing and community-based information sessions on relevant topics such as nutrition and meal planning, diabetes education, exercise, addiction education and pharmacy advice.
Who is this program for?
The Psychosocial Physical Health Pilot Program is for people living with mental health concerns who want to improve their physical health, and already receiving mental health support from a psychosocial or other mental health service.
This program could be right for you if you want to improve your physical health to help enhance your overall wellbeing.
In addition, this program aims to support the following priority groups, including people who:
- Identify as First Nations
- Are elderly individuals
- Have been referred through the Murrumbidgee Care Finder initiative
- Are currently linked to the Commonwealth Psychosocial Support Program (CPSP) in the Murrumbidgee region
- Have met NDIS access requirements but are yet to activate their plan or connect with services – these individuals can receive support through the program while awaiting their NDIS support
We provide this program in the Wagga Wagga area, including surrounding areas (within 100km).
How do I access the Psychosocial Physical Health Pilot Program?
To access the program, you must:
- Be aged 18 or over and have goals related to improving your physical health
- Be able to participate in regular sessions and access community-based support
- Be receiving mental health support in community.
GP referrals are not necessary – anyone can make a referral, just give our friendly team a call if you or someone you know who could do with some support.
This is a pilot program, available until 30 June 2026.
