Our team of healthcare professionals and specialists provide a range of free treatments to support management of your chronic condition, while our Aboriginal health workers work with you to coordinate appointments and social activities to connect with your community.


Who is the ICD clinic for?

The ICD Clinic is for First Nations people who need support with their chronic health needs. We deliver the following services through the weekly clinic:

  • Diabetes education
  • Nutrition support
  • Endocrinology
  • Exercise physiology
  • Podiatry
  • Respiratory physician
  • A gentle fitness and exercise class (single or group sessions) every Thursday from 2pm

We also host a range of social and educational activities such as morning/afternoon tea, cooking classes and a chance to catch up with friends and community.


How do I refer into the program?

The ICD clinic is open on Thursdays at our Bathurst office (102 Keppel Street).

It’s free, but you will need a referral from your doctor to access diabetes education, dietetics, endocrinology or respiratory services.

For podiatry services, it’s preferable for you to have a Chronic Disease Management Plan completed by your GP, with podiatry visits allocated.

A written referral stating you’re medically able to participate in physical activity is required to access exercise physiology services.
