Care Finders support vulnerable older people who would not be able to arrange services without intensive support, and don’t have a family member, friend or someone they trust who can help. If this sounds like a role you would excel in then we want to hear from you!

The Care Finder role:

  • Provides specialist and intensive assistance to help people understand and access aged care
  • Helps remove barriers for aged care recipients in connecting with relevant community supports
  • Supports improved integration between health, aged care and other systems at the local level
  • Connects and engages older people who have significant difficulty accessing aged care services.
  • Communicates and collaborates with participants, carers/families and relevant stakeholders.

Our ideal candidate will have the following:

  • Qualifications in Allied Health, Nursing, Community Services, Social Work/Social Science or equivalent
  • Working knowledge of the ageing process and related life transitions
  • Understanding of aged care supports and services available in the local and regional communities
  • Commitment to delivering a person-centred approach that respects and responds to each person’s individual needs preferences, values and life experiences
  • Well-developed communication and interpersonal skills and ability to engage and communicate effectively with people from a range of backgrounds
  • Strong solution focused, problem-solving skills
  • Strong administrative skills, including an ability to use and accurately enter information into client management systems, databases and/or portals.
  • Current Australian Drivers Licence and a willingness to travel occasionally for work purposes.
