We are seeking a dedicated Clinical Care Coordinator to join our Primary Health Team. This role is pivotal in delivering integrated, person-centred care for clients with complex health and social needs. Working collaboratively with clients, families, health professionals, and community services, you will ensure seamless care coordination and support.
This position offers flexibility and scope for either a Social Worker or Registered Nurse, adapting to changing program priorities and service needs.
About the Role:
In this role you will:
· Provide care coordination for clients with chronic and complex health conditions, ensuring timely access to appropriate services.
· Conduct comprehensive assessments to identify health, social, and psychosocial needs.
· Develop, implement, and monitor individualised care plans in collaboration with clients and their support networks.
· Facilitate referrals and linkages to health, community, and social services.
· Support clients in navigating the health system and advocate for their needs.
· Deliver client-centred education and self-management support to clients and families.
· Work collaboratively with multidisciplinary teams to ensure integrated care.
· Maintain accurate and timely documentation in accordance with organisational and professional standards.
· Participate in quality improvement activities and contribute to service development.
· Provide clinical supervision and support to students and junior staff as required.
· Uphold confidentiality and adhere to our Clinical Governance Framework.
About You:
To succeed in this role, you will have:
- Tertiary qualifications in Social Work or Nursing, with current registration (AHPRA or AASW Registration).
- Experience in care coordination for clients with chronic and complex health conditions.
- Strong understanding of health systems and community services.
- Excellent communication, advocacy, and interpersonal skills.
- Ability to work collaboratively within a multidisciplinary team.
- Commitment to person-centred care and continuous improvement.
