đź’ˇ About the Role
As Team Lead, you’ll provide clinical leadership and day‑to‑day support to our Care Coordination team, while managing your own care coordination caseload. You’ll help ensure services are high‑quality, client‑centred, and integrated with general practice and community partners.
This is a hands‑on leadership role where your expertise helps people navigate the health system, build health literacy, and stay well — and out of hospital.
🚀 What You’ll Be Doing
- Lead, mentor and support a team of Care Coordinators delivering complex, integrated care
- Provide clinical guidance on participant assessment, care planning and pathways
- Oversee referrals, intake, waitlists and service flow
- Ensure services meet organisational policies, funding requirements and quality standards
- Collaborate with senior leaders on reporting, audits and continuous improvement
- Build strong partnerships with GPs, health services and community organisations
- Promote the program across the health sector and community
- Keep your clinical skills sharp with your own care coordination caseload
👩‍⚕️👨‍⚕️ About You
You’re an experienced health or human services professional who:
- Has relevant tertiary qualifications (e.g. nursing, social work, health, community or human services)
- Is registered with AHPRA or eligible for AASW membership (as relevant)
- Brings strong operational and clinical leadership experience
- Has strong experience in integrated, person‑centred health care
- Can confidently assess complex needs and develop meaningful care pathways
- Communicates clearly, compassionately and inclusively
- Thrives in collaboration and relationship‑building
- Is organised, adaptable and comfortable with data, systems and reporting
Bonus points if you have experience in community‑based services or telehealth delivery.
