Team Lead – Care Finder

Marathon Health is seeking an experienced and values-driven Team Lead-  Care Finder to guide and support our Care Finder team while continuing hands-on work with clients. This is a unique opportunity to combine leadership, clinical expertise, and community impact in a role that genuinely changes lives.

Working on Wiradjuri Country, you’ll help ensure vulnerable older people—especially those without family or trusted supports—can navigate the aged care system with confidence, dignity, and care.

 About the Role

As the Team Lead Care Finder, you’ll provide day-to-day leadership and clinical guidance to Care Finder staff while carrying your own caseload. You’ll play a key role in ensuring high-quality service delivery across Western NSW and supporting the professional growth of your team.

Key responsibilities include:

  • Leading, mentoring, and supervising Care Finder staff to deliver high-quality, person-centred services
  • Providing clinical expertise and guidance on intake, referrals, and complex care navigation
  • Supporting older people to understand and access aged care services through My Aged Care
  • Strengthening connections between health, aged care, and community services
  • Monitoring service quality through audits, reviews, and continuous improvement
  • Supporting referral demand, waitlists, reporting, and contractual requirements
  • Promoting the Care Finder program across community and stakeholder networks
  • Working collaboratively with internal teams, community partners, and service providers

About You

You’re a confident communicator and supportive leader who thrives in community-based work. You bring compassion, professionalism, and a strong commitment to equity and access.

You’ll bring:

  • Qualifications in Social Work, Allied Health, Community Services, Nursing, Health, or equivalent experience
  • Sound knowledge of aged care services and the My Aged Care system
  • Experience supporting people with complex needs in community settings
  • A person-centred, culturally safe, and trauma-informed approach
  • Strong leadership, mentoring, and problem-solving skills
  • Excellent communication and relationship-building abilities
  • Strong administrative skills and confidence using client management systems
  • A current Australian driver’s licence and ability to travel across the region
  • Relevant checks (National Police Check, WWCC, NDIS screening) and eligibility to work in Australia

·        Experience supervising staff or managing intake and referral processes will be highly regarded.
