As a Care Finder, you’ll provide vital support to those who face challenges arranging services on their own—especially individuals without family or trusted friends to assist.
You’ll have the opportunity to be a change maker and contribute to the provision of aged-care services in your community!
This is a full-time position (75 hours per fortnight) where your skills and compassion can make a life-changing impact.

The Care Finder Role:
As a Care Finder, you will:
- Provide specialised, intensive assistance to help people navigate and access aged care services.
- Break down barriers for recipients, connecting them with vital community supports.
- Improve integration between health, aged care, and other local systems.
- Engage with older individuals facing significant challenges in accessing aged care services.
- Collaborate and communicate effectively with participants, their families, and relevant stakeholders.
Our Ideal Candidate Will Have:
- Qualifications in Allied Health, Nursing, Community Services, Social Work, or equivalent experience ….you may be an aged care case manger or package manager.
- Knowledge of local and regional aged care supports and services.
- A commitment to a person-centred approach that respects individual needs and experiences.
- Well-developed communication and interpersonal skills, with the ability to connect with diverse backgrounds.
- Strong problem-solving abilities and a solutions-focused mindset.
- Proficient administrative skills, including experience with client management systems and databases.
- A current Australian driver’s licence and a willingness to travel
