How can the care finder program help?

Care finders support vulnerable older people who need extra help to access aged care, find the system confusing or overwhelming, or do not have family, friends or someone they trust to help them.

Care finders work alongside you to make aged care easier to understand and access. They can help you to:

  • Talk to My Aged Care and arrange an assessment
  • Get support during your assessment
  • Find aged care providers in your local area
  • Understand your service options
  • Fill out forms and understand service agreements
  • Check in after services start to make sure things are going well
  • Connect with other community supports, such as health services, mental health support, housing or homelessness services, drug and alcohol services, and community and social groups

“You have no idea what you have done for my father and myself – the difference you have made in our lives.”
