Our diabetes management and support services are provided by our diabetes educators. They are here to help you understand your diabetes and make important changes that fit your daily life and support your wellbeing.


Type 2 and You information sessions

Our Type 2 and You information sessions are here to help you feel more confident and informed.

If you’re living with Type 2 diabetes, or supporting someone who is, then these sessions are for you.

Join us your way:

  • In person
  • Online from the comfort of home

However you take part, you’ll get clear, practical information and supportive guidance to help you manage Type 2 diabetes.

How can a diabetes educator help?

If you need help understanding or managing your diabetes, our team of Credentialled Diabetes Educators are here to support you. Our diabetes educators work with people who are living with pre-diabetes or type 2 diabetes. They can explain how diabetes affects the body, and what you can do to manage it day to day.

A diabetes educator can help you:

  • Understand your blood sugar levels
  • Learn what affects your blood sugar
  • Build confidence managing diabetes at home
  • Understand diabetes medicines
  • Set realistic health goals

The aim is to support you to live well with type 2 diabetes and reduce the risk of complications over time.


Who is diabetes management and support for?

This service is for:

  • Adults diagnosed with Type 2 diabetes
  • Adults with pre‑diabetes
  • Adults at risk of developing Type 2 diabetes

We use a person‑centred approach. This means the support is based on your needs, your health goals and what matters most to you.

Support may include help with:

  • Understanding blood sugar levels
  • Identifying risk factors
  • Making food and lifestyle changes
  • Building healthy habits over time
  • Looking after your emotional wellbeing

Diabetes affects everyone differently. Our team will work with you to find strategies that suit you.


How do I access diabetes management and support?

We currently offer two fully funded diabetes services:

Fully funded services for First Nations people

Available:

  • In Bathurst through the Indigenous Chronic Disease clinic
  • In Coonamble, Dubbo, Gilgandra and Wellington through local Aboriginal Medical Services. Please contact your local Aboriginal Medical Service directly to make an appointment

Fully funded services for community members living with or at risk of, type 2 diabetes

Fully funded Diabetes Education/Nutrition services are commissioned by the Rural Doctors Network (RDN) Medical Outreach Indigenous Chronic Disease Program and Rural Health Outreach Fund, and Western NSW Primary Health Network (WNSWPHN) Keeping Healthy Puura Manti Program and funded by the Australian Government Department of Health.
