Available for eligible Westfund members, the Westfund Care Coordination Pilot Program provides one-on-one help to access the right healthcare services and local supports tailored to your overall wellbeing.

Marathon Health Care Coordinators can help you by providing:

  • Health coaching to build your understanding of your health wellbeing needs, including support for lifestyle changes where needed
  • Guidance to navigate the healthcare system to find the right care for you
  • Support to reduce unnecessary medical costs and minimise avoidable repeat hospital admissions
  • Help to access Government funding and assistance schemes suitable for your ongoing health needs
  • Assistance to establish connections with local supports to enable you to live well in your community

Who is the Westfund Care Coordination Pilot Program for?

The program is a pilot program with limited places available to eligible Westfund customers aged between 18 and 75 years*. In addition, participants must be living with one or more chronic conditions, including, but not limited to:

  • Hypertension
  • Arthritis
  • Obesity
  • Mental health conditions
  • Cancer
  • Heart disease
  • Diabetes

*Children aged under 18 years and individuals aged between 76-80 years may be considered for this program with prior approval from Marathon Health.


Marathon Health has partnered with Westfund Health Insurance to provide the Westfund Care Coordination Pilot Program – available for eligible Westfund members.
