What is care coordination?

Care coordination means making sure your healthcare works together around you.

You’ll be supported by a dedicated Care Coordinator who will help you:

  • Understand your health and your options
  • Set personal health and wellbeing goals
  • Connect with the right services and supports
  • Build confidence to manage your health independently

What support will I receive?

If you join the program, your Care Coordinator will work with you to provide:

Personalised support

  • One-on-one conversations to understand your needs
  • A tailored care plan based on your goals
  • Regular check-ins to support your progress

Help navigating the health system

  • Coordination with your GP, specialists, or allied health providers
  • Referrals to services such as mental health, dietetics, or community programs
  • Support to access programs like My Aged Care and other services

Health coaching and education

  • Easy-to-understand information about your health
  • Support to manage your condition(s)
  • Help building healthy habits (such as activity, nutrition, or quitting smoking)

Advocacy and practical support

  • Help preparing for appointments
  • Support to overcome barriers like access, understanding, or confidence

How is the Westfund Care Coordination Pilot Program delivered?

Most support is provided by phone or video, so you can access care wherever you live.

In some cases, face-to-face support may be available depending on your location.


Who is eligible for the Westfund Care Coordination Pilot Program?

You may be eligible if you:

  • Are a Westfund member with Silver or Gold hospital cover
  • Are aged 18-75 years*
  • Are living with one or more of the following conditions:
    • Diabetes
    • Heart disease
    • Asthma
    • Arthritis
    • High blood pressure
    • Cancer
    • Obesity
    • Mental health conditions

You may also benefit if you:

  • Need help understanding your health or navigating services
  • Want support to reach your health goals
  • Find it hard to access or coordinate care

*People aged under 18 or 76–80 years may be considered with prior approval from Marathon Health.


What can I expect from the Westfund Care Coordination Pilot Program?

Your journey through the program may include:

  1. Referral – Westfund identifies you as suitable
  2. Initial contact – We explain the program and answer questions
  3. Assessment – We talk about your health, goals, and needs
  4. Care planning – You create a plan with your Care Coordinator
  5. Ongoing support – Regular check-ins, referrals and coaching
  6. Progress review – Tracking your goals and progress
  7. Completion – Transitioning to ongoing supports

Most people stay in the program for around 3-6 months, depending on their needs.

How do I get started?

You don’t need to apply directly. Simply follow the steps below to get involved.

Step 1: Referral – Westfund will assess your eligibility and refer you.

Step 2: We contact you – A Marathon Health team member will:

  • Explain the program
  • Answer your questions
  • Ask for your consent

Step 3: Get started – Once you agree, you’ll begin working with your Care Coordinator.

  • Your information stays private and secure
  • You can leave the program at any time
